I use Fabrik for all forms and data retrieval ,it will do all you want and more. Drawback it has a steep learning curve, there is a good forum and wiki to learn from. They have just released their stable version 4 guaranteed for J4 and I am using it on J5.0.2 with no issues. "O" nearly forgot its free....
